  17/09/25 - 09/01/26 (Week 1 - Week 17) Tai Tong En (0363164) Industrial Training: Projects/Tasks & Reflection INSTRUCTIONS There are 3 things required from you throughout your internship:   1. Internship Weekly Log - you need to fill this up on a daily basis, every week and get your company supervisor to sign it off as a form of verification approval. 2. Appraisal Form - this is the final evaluation of your placement from your company supervisor. ONLY HAND THIS TO YOUR SUPERVISOR 2-3 WEEKS BEFORE YOU FINISH. Your supervisor then can either pass it back to you to submit it or email it to me (if they do not want you to see their evaluation of you). 3.
